I use the smallest diamond bit for a dremel tool.  Then carve a slot for a small screwdriver blade.  Works everytime.  Has'nt failed me yet like other methods.  

I have found the easy out sometimes to "moosh" the brass outwards which makes it harder to turn.

Carl's "fishtail" pointed screwdriver is a good thing to try first.

I'm no expert, these are just things that have worked well for me (including the

adigurski.
