Whether you build a temperament from C or from A depends on which fork you decide to use. Choosing an octave in the middle of the keyboard makes the beats easier to hear, but which octave is chosen is an arbitrary decision. People customarily use just one temperament sequence because they get so familiar with it. Sequences using thirds and sixths along with fourths and fifths, instead of ONLY fourths and fifths, tend to be more accurate. The faster-beating intervals act like a vernier adjustment, and more tests are possible if one gets stuck. But acceptable work can still be produced with a fourths-and-fifths temperament given good ears and a lot of practice. If you are familiar with the intervals and beat rates and how they relate to each other, you can start on any note and end up with the same result.
