Do you use ear plugs?
I use -12db ones from Pianotech, they cut down on ear 'strain', from excess noise.

  I think Tom means that by focusing on the visual for that first 20 minutes requires very little attention to the ears. I definitely benefit in that regard. I remember when first learning to listen to overtomes and beats, for a while I thought I was going deaf! I heard this wooshing sound all day. As weeks and months went on, I adjusted. I think I was just bringing more awareness to the hearing section of the brain/body. But my ears still get stressed if tuning several pianos aurally in one day. So the ETD takes the edge off.
