Hmmm. Yes, I think MAYBE you have it! Loose flange pin (sorta).

The top part of the underlever system (what do you call it - the piece with the sostenuto tab and the damper wire screw. In fact, what do you call the three parts - the flange (obviously), the ?underlever? (with the lead pieces) and the subject piece)? The pin is properly in place - the pin is NOT loose (so maybe you don't get credit?). But the bushings are super compacted on both sided, allowing a huge (relative to most flange holes) hole for the pins ends to bang around in. 

How could this have occurred? Burr of some sort on end of pin that caused felt to wear out? Both ends? Any ideas?
