Consider this: The easiest thing to convert from piano to piano is
something that doesn't attach to the piano at all. Picture an inverted T
with an asymmetric cross, with the long leg bent perpendicular to both the
stem, and the cross. The stem of the T is about 50cm long and straps to the
inside of the right leg, with appropriate padded forms. The foot sits on
the short side of the cross, about 12cm long, with a 5cm spacer under the
outboard end as a pivot. The long side of the cross, extending about 10cm
left and then forward about 18cm, has a padded inverted U that straddles
the damper pedal. Rocking the leg left depresses the pedal. The leverage
ratio is negotiable by modifying the lever dimensions to suit during
fabrication, or could be made adjustable at a sacrifice to simplicity and
durability.

That is a good idea if the lateral strain on the thigh muscles don't get him.
I thought of a pedal extender. This would function by lifting and lowering
the leg. Perhaps the sheer weight of the leg on the 'lever' will operate
the pedal and by lifting the leg the pedal is released.

Plywood fashioned in a U-pattern.

The notch would slip over any pedal and wedge under the lyre or bottom board
and the weight of the leg will depress the platform.

Make it a bi-level rig so that the leg isn't raised too high hitting the 
keybed:

Rather than utilizing the ball of the foot and calf muscles, the thigh 
muscles are employed.

Transferrable to any piano.
