Bursitis

Such pain may be caused by bone spurs in or around the rotator
cuff.  Bursitis is an inflammation of the bursa, the sheath holding
muscles in bundles.  If anti- inflamatories don't help then it may
well not be an inflammation.  Bone spurs can cause a LOT of pain
but are easily removed by a bone surgeon, better than a "normal"
surgeon.  Out of commission for a week or two then back to "normal"

I am having the same problem and will arrange to get x-rays or MRI
in the immediate future to see if it IS bone spurs.  

Bursitis can be cured with medrol but it is no longer given to
humans because it causes diabetes.  

You do need to see a specialist, neurologist or orthopedist.
