I've had the slap of jack against hammer flange recently. Only occurred
on fortissimo, but was the devil to diagnose. It certainly didn't look
like it would happen with the frame/action out on the bench. I chose to
cure by a little of everything, particularly:
reduce keydip on offending keys just a tad
lower hammers just a tad
shave a bit of wood off the bottom of the flange tongues
tell the pianist not to play so darned loud (Hah!)
	Less spongy front rail punchings would also help. I did a couple more
things, but disremember at the moment.
