IMO the weirder the problem the more I think it came from the factory that way. Maybe the wire was 
initially installed just on the edge of the hole.....
But I suppose a certain cyclic action could float the wire just right, hmm maybe.
I worked on a grand recently that one key was missing all it's key weights, the holes were so clean 
and neat that I think it never had them.
