Piano Appraisal

I will be doing my first piano appraisal real soon.  The gentleman that called me, wants me to go the piano store and look at a Steinway Pianola.  He needs the following items:
   
  1.  Current Condition.
  2.  Appraisal.
  3.  Value after Restoration.
  4.  Professional Opinion.
   
  Do any of you have specific forms that you use for this information?  What have you learned from experience when dealing with piano stores?
   
  My goal here is to surpass this gentleman's expectations.
