  We inform them of the potential  charge on the phone  prior to the 
appointment.  No surprises is a good policy.

Anytime I consider the piano not worth tuning, I  just charge for a service 
call.
That is of course after informing them,  why.  I would also explain the 
options, if any, and what the costs would  be.
