    I use generic forms and imprint my name and address at the top of =
each with a rubber stamp.  Carefully done, they look fine.  Its less =
expensive than having them printed, you can make up only as many as you =
need at a time, and if anything changes on the heading, you're not stuck =
with the wrong information on printed bills, just get a new stamp made.  =
BTW, with self-inking stamps if you take an old one in, they can put new =
lettering on your stamp-cuts the cost in half. =20
