Billing Slips - where do ya get 'em?

I never did get around to getting the printed billing slips.
I just get the mass produced books from Staples. I had a rubber stamp made up with my name, address, telephone # ( an 800 number is worthwhile, if much of your work is long distance), cell phone number and my registration number for the sales tax.
It looks reasonably professional, and over the years, I have changed the basic book or stamp, to match my needs.
