Metal fatigue.  Bend wire often enough it breaks.  

Once began there is no recovery.

Short and long term solution is to restring the bass with larger
tuning pins.  Get them from Yamaha, they have the exact size pins for
this job.

At 5% relative humidity the pinblock and other wood structures may
very well be severely damaged.  Check for cracks, delaminations and
loose glue joints throughout the piano.

These are the penalty for piano abuse.
