Patrick,

One other thing to be sure and check for. Be absolutely certain that there is
no separation of the bridge cap from the base of the bridge.  You've probably
already checked, but this can certainly cause a "dead note" and most often
occurs at a cut-out in the bridge (at least in my experience.)
